Health Insurance Benefits
All full-time employees working at least 30 hours per week and their eligible dependents qualify for benefits on the first day of the month following 60 days of employment with the company. Your health coverage premiums are paid 70% by the company. The company currently offers a High Deductible Health Plan (HDHP) with Blue Cross Blue Shield of NM (BCBSNM). If you enroll with the BCBSNM High Deductible Health Plan, you are eligible to participate in the Health Savings Account (HSA) to help pay your health insurance deductible and qualified medical expenses. The employee owns this account, but employees and company can contribute funds for the duration of your employment with the company. For more information, you can go to www.healthequity.com.
The company also offers a Health Reimbursement Account (HRA). The HRA will reimburse up to 50% of the deductible after the employee has met the first half of the deductible. Employees will only be covered for medical expenses up to the amount credited. Employees will be reimbursed after submitting proper documentation to the plan administrator.
401K Retirement & Savings Plan
After six months of full time, employment employees are eligible to participate in the Envirotech 401K Plan. Currently, Envirotech matches up to 3% with a graduated vesting schedule.
The 401K plan allows you to set aside pre-tax contributions for retirement. The Roth 401k allows you to contribute after-tax dollars, but qualified withdrawals are tax-free.

Professional Development Assistance
Employees are encouraged to seek educational enrichment to further enhance their capabilities and career development. Active, full-time employees are eligible for partial reimbursement for tuition and fees upon completion of any course related to their current or future position. The course must be approved by management in advance to be eligible.
Dental & Vision Benefits
You are eligible for dental and vision benefits within your first 90 days of employment with the company. Employees are responsible for 100% of the premiums and those will be payroll deducted bi-weekly. Currently, dental coverage is with Delta Dental of NM and vision coverage is with VSP. Life Accidental & Supplement Insurance Coverage
The company provides a $25,000 basic life insurance policy for all eligible employees. This policy is paid 100% by the company.
Employees can purchase additional life insurance of 1 to 5 times their basic annual earnings up to a maximum of $500,000. Guaranteed policy issue is $70,000. Dependent coverage is also available. Coverage for dependents cannot exceed the employee life amount: spouse increments of $5,000 up to $500,000, spouse guarantee policy issue is $25,000, child (6 months – 26 years old if student) increments of $2,000 to $10,000.

All eligible employees can purchase supplemental insurance coverage with AFLAC. Envirotech pays 50% of the accident policy premium; all other policies are 100% employee funded. For more information, you can go to www.aflac.com.
Vacation
All full-time employees will begin accruing vacation at date of hire. See the schedule for Vacation Accruals.
Holidays
All full-time employees are eligible for 7 paid holidays after 90 days of employment.
Employment
Envirotech is a drug-free workplace. All new hire or rehire applicants given an offer of employment must submit to pre-employment drug screening. Additionally, random drug screenings and post-accident screenings are performed.
All applicants must complete a background check, motor vehicle record check, and reference check.
Employees must meet the driver qualifications for company insurance requirements and provide evidence of a valid New Mexico driver’s license or equivalent with a clean driving record.
New employees must complete the Form I-9 on the first day of employment with acceptable documents that establish identity. Employers must complete Form I-9 to document verification of the identity and employment authorization of each employee (both citizen and non-citizen) hired after 11/6/86 to work in the United States.
Envirotech employees are paid every two weeks on a Friday. All employees must provide proper supporting documentation for direct deposit setup and maintain current information for the duration of employment.
